  • Proficient in medication management for elderly patients, with attention to polypharmacy risks and medication reconciliation.
  • Hands-on skill with electronic health records to effectively document, track, and manage patient information securely and accurately.
  • Excellent communication and interpersonal skills to collaborate with healthcare teams and build trust with elderly patients and families.

Roles and Responsibilities
