- Conducting patient onboarding calls to understand their goals and preferences
- Coordinate the care of patients with the clinical team, including scheduling appointments, managing medications, and facilitating communication between patients and the clinical team.
- Educate patients and families about pain management techniques and strategies, facilitate medication management, care sessions, lifestyle modification and self-care techniques
- Maintain accurate and up-to-date patient records, including documentation of care plans and progress notes.
- Gather and organize care program feedback from patients and the clinical team
- Actively participate in improving the program's protocols, systems and processes to have better patient and provider outcomes.
- 0-1 years of consumer-facing hospitality experience as health coach/Habit coach/Air hostess/Front desk attendant.

- Excellent process orientation and management (including and not limited to maintenance of data & critical records).
- Strong Troubleshooting & Problem-solving skills
- Multi-tasking and time-management skills, with the ability to prioritise tasks
